APPLICATION FOR SCHOLARSHIP

To be considered as a recipient of the Chester E. and Marion D. Groth Scholarship, please fill out the following application completely and mail it along with your most recent official high school transcript. Three letters of recommendation are required using the enclosed letter of recommendation questionnaire.

APPLICATIONS MUST BE RECEIVED in the church office prior to or no later than noon, Friday, February 29, 2008. Applications sent by fax and e-mail will not be accepted.

Note:

· Applicants must be a graduating high school senior from a Minnesota high school

· Be entering college as a freshman

If you are selected for an audition, you must:

1) provide your own accompanist.

2) play for 5 – 10 minutes (two contrasting pieces).

3) memory is not required.
